Where does server.cfg go on a Garry's Mod server?

The command line. The gamemode and starting map are read before server.cfg runs, so setting them there is too late. Use +gamemode and +map on the launch line, which is why this generator produces both the config and the command.

Why is my rcon password not working?

Two common causes. If rcon_password is the same as sv_password then everyone who can join also holds admin, which some hosts refuse outright. And if the value contains a space or a quote it has to be quoted properly or srcds reads only the first word.

How many slots should a Garry's Mod server have?

Slots come from -maxplayers on the launch line, not from server.cfg. What limits it in practice is memory: a plain Sandbox server is light, but a DarkRP or TTT server with a full Workshop collection wants around 4 GB, and a busy community server with heavy addons wants 8 GB.
